Ontario man can't collect lottery prize after he forgets where he bought ticket

Except he actually collected his winnings.
If these answers are not provided, the claim then undergoes further review, which extends the time a winner must wait in order to receive their prize.

So the prize was delayed, not denied. Misleading headlines like this really make it difficult to trust the source.
Surely the lottery could tell him?
- It's part of the security test to make sure you are the actual owner of the ticket.
  
  Where I live, there's place on the lottery ticket for you to fill in your name, ID number, and phone number.
  
  If you leave it blank and lost the ticket, then whoever posses the ticket have legal claim to the price.
- Looks like they asked him as a method of identity verification.
